The growing number of older adults with dementia is increasingly being met with technological interventions. Among them are GPS-tracking devices that assist people with dementia (PwD) with orientation. The extent to which this technology should be utilized, and how to balance safety, mobility, and privacy has been the subject of extensive scholarly debate. To evaluate the range of normative attitudes of lay people towards the issue, we conducted an online citizens’ jury (CJ) with 17 German citizens aged 18 to 67. To examine the relationship between individual perspectives and collective attitudes, we further conducted qualitative interviews with participants. As part of the CJ process, participants collaboratively authored a comprehensive policy proposal containing nuanced views on ethical and technical aspects. Despite differing perspectives, particularly on the meaning of autonomy, participants reached broad consensus on issues such as informed consent, data protection, and device customization, and were able to agree to disagree on more contentious points. Our analysis demonstrates the capacity of lay participants to engage in ethical reflection, achieve consensus, and navigate disagreement. We further reflect on methodological considerations and examine the contribution lay deliberation can provide to contemporary bioethical debates. The online CJ format enabled in-depth, structured deliberation among diverse lay participants on a complex bioethical topic. The use of video conferencing software allowed for wide geographic participation. Combining the CJ with qualitative interviews and field notes provided rich insight into both collective and individual normative attitudes.

Current challenges in medicine and healthcare raise new questions regarding the moral relations between generations, thus highlighting the increasing relevance of intergenerational perspectives in healthcare ethics. However, the underlying notions of generations often remain vague and heterogeneous. This contribution aims to clarify the scope of conceptual meanings of ‘generation’ through explication and differentiation in order to advance the analytical potential of intergenerational perspectives in healthcare ethics. We argue that the concept of generations needs theoretical elaboration with regard to the dimensions of collectivity and temporality. We first introduce three approaches towards the theoretical conceptualization of generations: a genealogical, a chronological, and a socio-cultural approach. Regardless of their differences, all three essentially share an understanding of generations as collectives situated in time. Accordingly, we then examine the scope of underlying notions of collectivity and temporality, touching upon fundamental ontological, epistemological, and moral philosophical implications. We distinguish a skeptical individualist, an aggregationist, and an entity view of collectivity, as well as a formal, linear, a subjective, existential-narrative, and a socio-cultural understanding of temporality. The combination of these dimensions allows the development of a systematic matrix of conceptions of generations and intergenerational relations in healthcare ethics whose analytical potential we illustrate with regard to three paradigmatic examples. We provide a systematic summary of our considerations and outline a research agenda that addresses desiderata for intergenerational perspectives in healthcare ethics, encompassing clinical ethics, research ethics, and public health ethics, as well as meta-ethical questions.

Erst im Zeichen der steigenden durchschnittlichen Lebenserwartung sowie des wachsenden Anteils älterer Menschen an der Gesamtbevölkerung rücken ethische Fragen des Alter(n)s in den letzten Jahrzehnten zunehmend in den Brennpunkt gesellschaftlich-politischer und akademischer Auseinandersetzungen. In der Angewandten Ethik betreffen die einschlägigen Debatten insbesondere die Bereiche der Gerontologie, Medizin und Gesundheitsversorgung, der sozialen Arbeit und Sozialpolitik sowie der Ökologie und Nachhaltigkeit.

Solidarity between generations served as a prominent but controversially discussed normative reference point in public debates about the Coronavirus-pandemic. The aim of this contribution is the empirical reconstruction and ethical evaluation of prominent notions of intergenerational solidarity and their underlying assumptions in the public media discourse on the pandemic in Germany. After a brief introduction to the concept of intergenerational solidarity and the pertinent discourses during the pandemic, we present the results of a comprehensive qualitative content analysis of 149 articles from leading media in Germany. On this basis, we carve out three typical understandings of intergenerational solidarity: (a) communal care, (b) mutual support, and (c) responsible use of freedom. We discuss these understandings and the underlying ‘moral paradigms’ and evaluate their theoretical and practical implications from an ethical point of view, drawing conclusions for discourses on future societal crises.

In the evaluation of genomic high-throughput technologies, the idea of “utility” plays an important role. The “clinical utility” of genomic data refers to the improvement of healthcare outcomes, its “personal utility” to benefits that go beyond healthcare purposes. Both concepts are contested. Moreover, there are only few empirical insights regarding their interpretation by those professionally involved or personally affected. Our paper presents results from qualitative research (20 semi-structured interviews) regarding professionals’ and personally affected people’s views on the utility of genomic information. We find that the discussion of clinical utility is the domain of professionals who primarily consider aspects of test validity and clinical outcomes. By contrast, personal utility plays a prominent role in affected persons’ discussions. Four different aspects are addressed: (a) life and family planning, (b) relief and justification, (c) self-knowledge and self-determination, and (d) entertainment. These differences have important implications for informed consent and counseling in the context of genomic high-throughput technologies.

Der vorliegende Schwerpunkt beschäftigt sich mit dem Zusammenhang von Zeit und gutem Leben. Er verfolgt das Ziel, die systematische Auseinandersetzung mit den Themen, Problemen und Fragestellungen voranzubringen, die das Verhältnis zwischen den zeitlichen Strukturen der menschlichen Existenz auf der einen und den Bedingungen guten, sowohl subjektiv erfüllten als auch objektiv gelingenden Lebens auf der anderen Seite betreffen. Die verschiedenen Beiträge spannen ein breites Spektrum theoretischer Ansätze, thematischer Ausrichtungen und praktischer Bezüge auf. Es reicht von der philosophischen Erörterung grundlegender Fragen der zeitlichen Verfasstheit guten Lebens über die Auseinandersetzung mit psychopathologischen Phänomenen gestörter Zeiterfahrung bis hin zur sozialund erziehungswissenschaftlich angeregten ethischen Beschäftigung mit konkreten Lebensphasen und praktischen Anwendungsfeldern.

The idea that dementia is essentially a return to childhood and those affected must somehow be similar to children constitutes a deeply rooted and pervasive cultural trope. While such tropes may be helpful in making sense of an otherwise elusive and inscrutable state, they can at the same time promote inadequate understandings of dementia and hence also influence our attitudes and behaviour towards those affected in several problematic ways. In the present work, we provide a detailed account of the origins and implications of the idea that people with dementia are somehow similar to children. In order to spell out the normative significance of this trope and to analyse its shortcomings, we will rely on a life‐course perspective. We will point out that the second‐childhood trope does not do justice to the particularities of the life phases of both children and people with dementia. This lack of biographical sensitivity brings about undignified treatment of and harm to those who have previously lived more autonomous lives. In conclusion, we will argue that people with dementia should not be equated with children as they are at a different point in their lives. This critical analysis of the second‐childhood trope shows the usefulness of a culturally and biographically sensitive theoretical framework for bioethical reflection. In particular, it provides reasons to reject infantilizing attitudes and behaviours and instead take seriously the personality, values and preferences shaped throughout the course of an individual's life.

This contribution sets out to criticize the prominent metaphor of “death while alive” in the context of dementia. We first explain the historical origin and development as well as the philosophical premises of the image. We then take a closer look at its implications for understanding dementia and societal attitudes and behaviours towards those affected. In doing so, we adopt a life course perspective that seeks to account for the ethical significance of the temporal extension and structure of human life. According to this perspective, individual existence in time is characterized by normative standards of age-appropriate behavior, evaluative standards of a good life, and teleological notions of successful development which require theoretical analysis and ethical discussion. Such a perspective can contribute significantly to spelling out the implications of the metaphor of death while alive and to criticizing their problematic aspects. Indeed, it makes clear that this metaphor aligns dementia with a different point in the human life course, thus ultimately framing it as a kind of deviation from the biographical norm, a disruption in an assumed temporal order of existence. At the same time, the life course perspective can help to understand why this conception involves ethically problematic distortions and blind spots. The resulting considerations allow conclusions with regard to medical and care ethical debates about self-determination, surrogate decision making, and advance directives in the context of dementia. Furthermore, on a theoretical-conceptual level, they also illustrate the importance of a biography- and culture-sensitive approach to philosophical and ethical reasoning in biomedicine and the life sciences.

Unter dem Begriff ›Anti-Aging‹ wird seit etwa drei Jahrzehnten eine wachsende Anzahl medizinischer wie außermedizinischer Präparate bzw. Verfahren konzipiert, entwickelt und eingesetzt, die allgemein der Bekämpfung des Alterns dienen sollen. Die so genannte Anti-Aging-Medizin will in den Alterungsprozess des menschlichen Körpers eingreifen, um ihn zu verlangsamen, aufzuhalten oder rückgängig zu machen. Dabei variieren die konkreten Angriffspunkte, Vorgehensweisen und Zielsetzungen der einzelnen Ansätze durchaus beträchtlich.

Angesichts der steigenden Lebenserwartung, der Auflösung traditioneller Sorgestrukturen sowie des Fachkräftemangels in der professionellen Pflege wird für die Lebensgestaltung im höheren Alter und die pflegerische Versorgung älterer Menschen verstärkt auf technische Lösungen gesetzt. Der Beitrag nähert sich der damit einhergehenden Entwicklung der Mensch-Technik- Interaktion im demographischen Wandel aus dem Blickwinkel der philosophischen Anthropologie. Ziel ist es, die Bedeutung anthropologischer Aspekte und Dimensionen für die Auseinandersetzung mit so genannten Gerotechnologien auszuloten. Dazu gebe ich zunächst einen orientierenden Überblick über das Spektrum und die Entwicklungsperspektiven technischer Assistenzsysteme für ältere Menschen. Im Anschluss wird die Rolle individueller und gesellschaftlicher Altersbilder in der Diskussion und Anwendung dieser Gerotechnologien beleuchtet. Vor diesem Hintergrund werden schließlich einige grundlegende anthropologische Fragen und Desiderate der Mensch-Technik-Interaktion im Alter angesprochen. Sie betreffen insbesondere die Gesichtspunkte der körperlichen Verfasstheit, der zeitlichen Erstreckung und Verlaufsstruktur sowie der relationalen Ausrichtung und Einbettung menschlichen Seins.

Bringing together scholars from philosophy, bioethics, law, sociology, and anthropology, this topical collection explores how innovations in the field of biomedicine and the life sciences are challenging and transforming traditional understandings of human temporality and of the temporal duration, extension and structure of human life. The contributions aim to expand the theoretical debate by highlighting the significance of time and human temporality in different discourses and practical contexts, and developing concrete, empirically informed, and culturally sensitive perspectives. The collection is structured around three main foci: the beginning of life, the middle of life, and later life. This structure facilitates an in-depth examination of specific technological and biographical contexts and at the same time allows an overarching comparison of relevant similarities and differences between life phases and fields of application.

Public and academic medical ethics debates surrounding justice and age discrimination often proceed from a problematic understanding of old age that ignores the diversity of older people. This article introduces the feminist perspective of intersectionality to medical ethical debates on aging and old age in order to analyze the structural discrimination of older people in medicine and health care. While current intersectional approaches in this field focus on race, gender, and sexuality, we thus set out to introduce aging and old age as an additional category that is becoming more relevant in the context of longer life expectancies and increasing population aging. We analyze three exemplary cases on the individual, institutional, and public health level, and argue that considering the intersections of old age with other social categories helps to accommodate the diverse identities of older people and detect inequality and structural discrimination.

Zusammenfassung Steigende Lebenserwartung, sozialer Wandel und medizinische Innovationen fordern traditionelle Sichtweisen auf das Alter(n) heraus. Was einst als eine „normale“ Alterserscheinung galt, wird heute im Lichte veränderter Lebensentwürfe und neuartiger Interventionsmöglichkeiten oft schon als Erkrankung aufgefasst und behandelt. Altersbezogene Gesundheitsstandards und Behandlungsziele geraten in Bewegung. Es eröffnet sich ein Spannungsfeld zwischen Bagatellisierung und Pathologisierung von Alterungsprozessen, das der ethischen Reflexion bedarf. Der Beitrag geht der Frage nach, wie individuelle und gesellschaftliche Vorstellungen des Alter(n)s im Kontext der modernen Medizin ethisch zu verstehen sind. Dazu geben wir zunächst einen Überblick zur Rolle von Altersbildern in der medizinischen und pflegerischen Versorgung älterer Menschen. Anschließend werden begrifflich-theoretische Perspektiven umrissen, die solche Bilder des Alter(n)s einer strebensethischen Analyse zugänglich machen. Welche Formen der Gesundheitsversorgung im höheren Alter als sinnvoll und angemessen zu gelten haben, ist demnach nicht zuletzt im Licht der Frage nach der Zeitstruktur guten Lebens zu diskutieren.

This book examines the relevance of modern medicine and healthcare in shaping the lives of elderly persons and ageing societies. Combining individual and social dimensions, Planning Later Life discusses the ethical, social, and political consequences of increasing life expectancies and demographic change in the context of biomedicine and public health. By focusing on the field of biomedicine and healthcare, the authors engage readers in a dialogue on the ethical and social implications of recent trends in dementia research and care, advance healthcare planning, or the rise of anti-ageing medicine and prevention. Bringing together the largely separated debates of individualist bioethics on the one hand, and public health ethics on the other, the volume deliberately considers the entanglements of envisioning, evaluating, and controlling individual and societal futures. The process of devising and exploring the various positive and negative visions and strategies related to later life is rarely reflected systematically from a philosophical, sociological, and ethical point of view. As such, this book will be crucial to those working and studying in the life sciences, the humanities, and the social sciences, particularly in the areas of bioethics, social work, gerontology and aging studies, healthcare and social service, sociology, social policy, and geography and population studies.

Der Perspektive der Betroffenen kommt im medizinethischen Fachdiskurs der Gegenwart eine grundlegende Bedeutung zu. Im Fall der Demenz wird der Zugang zu ihr allerdings durch krankheitsbedingte Abbauprozesse zunehmend erschwert. Neben anderen künstlerisch-ästhetischen Annäherungen ist in den letzten 15 Jahren auch eine Fülle an Spielfilmen zu verzeichnen, die sich mit der Erfahrung der Demenz beschäftigen. Der Beitrag geht der Frage nach, inwieweit solche filmischen Gestaltungen neue Zugänge zum Demenzerleben eröffnen und was Film als Medium und Kunstform somit für die ethische Auseinandersetzung mit Demenz zu leisten vermag. Dabei werden drei prominente filmästhetische Zugangswege eingehender untersucht, an ausgewählten Beispielen erläutert und unter ethischen Gesichtspunkten erörtert. Es zeigt sich, dass die ethischen Potenziale eines reflektierten Umgangs mit filmischen Annäherungen an die Demenz längst nicht ausgeschöpft sind.

ZusammenfassungDie wachsende Nachfrage nach Anti-Aging-Medizin wirft die Frage auf, welche medizinischen Leistungen ein solidarisches Gesundheitssystem tragen sollte. Die deutsche Entscheidungspraxis beruft sich auf den Begriff der Krankheit. Im Blick auf Anti-Aging wäre demnach 1) zu klären, was der Krankheitsbegriff bedeutet, 2) zu prüfen, ob das Altern sich unter diesen Begriff subsumieren lässt, um 3) abzuleiten, inwieweit Anti-Aging-Maßnahmen zur Verfügung zu stellen sind. Dieses Prozedere führt jedoch zu keinem brauchbaren Ergebnis. Unter Berufung auf den Krankheitsbegriff allein ist der Umfang solidarischer Gesundheitsversorgung nicht zu bestimmen. Allerdings lässt sich durch Reflexion auftretender Probleme ein gangbarer Entscheidungsweg aufzeigen. Er sieht eine faire Verständigung über die Frage vor, wie wir alt werden wollen.

Ageing has become a central topic of medical ethical debates, e.g. regarding autonomy and care in geriatric practice, the just distribution of healthcare or the implications of anti-ageing medicine. In all these debates, however, particular conceptions of ageing are tacitly presupposed. The aim of my research is to develop an explicit understanding of the relevance of ageing for ethical reasoning, providing a conceptual framework for discussing concrete medical ethical problems in a more comprehensive and reflected manner. I proceed from the life course approach, assuming that our life is shaped by a time regime defined and implemented by social institutions and structures, reflected in cultural images, and internalized as an “inner clock” directing individual decisions. From the ethical viewpoint, two concepts appear particularly significant: In a deontological perspective, the idea of “age norms”, social standards of age-appropriate behavior, becomes relevant. In a teleological perspective focusing on individual flourishing and good life, the concept of life plans, comprising age-related self-images and expectations, is important. I will briefly sketch the current medical ethical debates around ageing, then delineate the life course perspective and its implications for ethical reasoning, and finally exemplify its theoretical potential regarding medical ethical controversies about long time care, resource allocation and life extension.
